AZOVA will confirm your vaccination record and create a Vaxigo electronic vaccination record with your Simple Health Pass and Credential ID.  

Your Credential ID enables you to share only your vaccination status without sharing your vaccination record and personal information. Enter your Credential ID when asked for entry to facilities, events and travel destinations or with any venue that requires third-party confirmation of your vaccine record. Your record is HIPAA, SOC2 and HITRUST compliant and secured on AZOVA.  

Complete the process for each person whose record needs to be confirmed. If you are an adult, you must create your own account and submit a request on your own behalf. If you are creating a record on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who the patient is. You can get as many records confirmed for as many dependents as needed.
